Connecting Care: A Chronic Disease Action Framework for Tasmania 2009 – 2013 charts the direction for a coordinated and strategic approach towards the better prevention, detection and management of chronic disease in Tasmania over the next four years.
The Framework sets out a shared vision, principles, goals and action areas that can guide all individuals and groups working to improve the health and wellbeing of Tasmanians.
Connecting Care has been developed in cooperation with an extensive network of clinicians, general practitioners, community sector organisations, researchers, health service managers and consumers from across Tasmania.
DHHS will continue this collaborative approach as we move to implement the Framework. A Chronic Disease Clinical Network is being established as an immediate priority and will oversee the development of an implementation plan for the Framework.
