The HACC Program Seeks Your Input

The Home and Community Care (HACC) Unit are seeking input from consumers and providers of HACC services on what we need to do to sustain and improve HACC service delivery over the next three years. All interested parties are invited to have their say. Please click here for further information.

Home and Community Care

In Tasmania the HACC Program is jointly funded by the Australian and Tasmanian Governments. HACC provides funding to both the government and non-government sectors to enable them to deliver a range of community care services.

The HACC Program is a key provider of community care services to frail aged people and younger people with disabilities, and their carers. The overall objective of the HACC Program is to enhance the independence of people in these groups and to avoid their premature admission to long term residential care.

Community Care services provided in Tasmania include:

  • domestic assistance;
  • social support;
  • community nursing;
  • allied health services;
  • delivered and centre based meals;
  • personal care;
  • home maintenance and modification;
  • respite;
  • assessment and/or referral services;
  • transport;
  • counselling, support, information and advocacy; and
  • case management and co-ordination.
