Cancer Care Coordinators
It is important people with cancer receive timely access to relevant treatment information and support services. To ensure each patient receives the necessary level of support, CanNET Tasmania established a trial Cancer Care Coordinator project for Tasmania. Cancer Care Coordinators ensure the continuity of care throughout a patient’s journey.
Two Cancer Care Coordinators have been appointed, one for the North of Tasmania and one for the south of Tasmania, both concentrating on Lung Cancer and Colorectal (bowel) Cancer.
Cancer Care Coordinators:
- Are a central point of contact for patients and their families
- Help patients navigate their way through treatments and the health system
- Can provide information, education and support to patients and families
- Help meet needs surrounding cancer, treatments, symptom management, support services, health care services
- Can refer patients to the most appropriate person or service to meet their needs
- Facilitate standardise approach to Multidisciplinary Team meetings
- Facilitate effective communication between Multidisciplinary Teams, GP’s, patients and their families
- Support both acute and ambulatory patients across services and geographic locations
- Ensure timely referral to relevant support and community services
- Act as patient advocate
- Are a primary contact point for all
- Ensure patients have equity of access to the benefits of coordinated, evidence based care
