Primary Health Services Plan
- Will provide a greater focus on health promotion, illness prevention and the management of chronic conditions in community health centres and rural health facilities.
- Will introduce a range of initiatives to improve the integration of primary health and acute care services including the development of Primary Health Partnerships, Integrated Care Centres and Clinical Networks.
- Will help community nurses to better support post acute care, health promotion and prevention, and a self-help approach to chronic disease management.
- Will develop a closer relationship with general practitioners to better respond to the challenge of chronic disease.
- Will provide additional funding for non-urgent transport for disadvantaged people ineligible for assistance under the Home and Community Care program.
