Primary Health Service Plan

  • Will provide a greater focus on health promotion, illness prevention and the management of chronic conditions in community health centres and rural health facilities.
  • Will introduce a range of initiatives to improve the integration of primary health and acute care services including the development of Primary Health Partnerships, Integrated Care Centres and Clinical Networks.
  • Will help community nurses to better support post acute care, health promotion and prevention, and a self-help approach to chronic disease management.
  • Will develop a closer relationship with general practitioners to better respond to the challenge of chronic disease.
  • Will provide additional funding for non-urgent transport for disadvantaged people ineligible for assistance under the Home and Community Care program.

    The Department of Health and Human Services is undertaking an Expression of Interest process to engage a consultant to provide an independent review of a proposal to establish a Multi Purpose Service for the Central Highlands.
