Community nurses work with people and their families to help prevent disease, maintain health and treat any existing health problems. This service aims to promote, support and maintain the patients independence, safety and healthy lifestyle, at the same time as providing assistance to carers.
Community Nurses provide a range of health programs within the community. These programs include support for clients after surgery, nursing care for clients with long term disability and/or chronic disease, and community health promotion programs. Community Nurses provide services in clients homes, and in clinic and community settings and work with individual clients, groups of clients and/or sectors of the community to promote, support and maintain healthy lifestyles.
