In Tasmania the Home and Community Care (HACC) Program is jointly funded by the Australian and Tasmanian Governments.  HACC provides funding to both the government and non-government sectors to enable them to deliver a range of community care services.

The HACC Program is a key provider of community care services to frail aged people and younger people with disabilities, and their carers.  The overall objective of the HACC Program is to enhance the independence of people in these groups and to avoid their premature admission to long term residential care.

The list below provides information on the Community Care services provided in Tasmania.
