Access to Records
You have the right to ask to see information kept about you by Government services.
Requests for access to your hospital records should be made directly to the Manager Patient Information Management Systems at the hospital that holds your patient file. This information is not available under the Freedom of Information Act 1991.
A fee may be incurred if you require copies.
Confidentiality of Records
Under the Personal Information Protection Act 2004 the Department of Health and Human Services is the custodian of personal information and the collection, use and disclosure of that information is governed by the Act.
