Our Model of Care

Our model of care is based on two key themes and eleven service principles:

Recovery Focused

We have a recovery focused model of care. We believe that Mental Health Services has an obligation to ensure that every effort is made to focus our services on the consumer’s recovery.

Assertive case management

Each consumer who is a client of Mental Health Services will have a case manager who will be responsible for coordinating the treatment and support that is provided to the consumer and their carers. They will intervene when appropriate and be accessible in a crisis. They will design interventions that will meet the goals that are set by the consumer and mobilise resources from across Mental Health Services and from the community as they are needed.

Our Service Principles

The following service principles are the foundation for the development and delivery of our services.

  • Single point accountability – we believe that every staff member in Mental Health Services has the responsibility to ensure that we deliver quality care. Mental Health Services clinicians will be accountable for the consumers they work with and will act as a central point of contact to help the consumer make the best possible use of the services available.
  • Customer focused care – we will keep the consumer and their world at the centre of the care and treatment we provide.
  • Partnerships – we believe that consumers must be involved as much as possible in all decisions about their care. We will also work collaboratively with other organisations in order to achieve the best possible outcomes for consumers, family members and carers.
  • Comprehensive Service – we are a multi-disciplinary service that proactively links to community organisations to ensure that the full range of consumer needs are met.
  • Integrated and standardised service – we will endeavour to provide consumers with easy access to a wide range of services and deliver these services in a seamless way to ensure continuity of care. Our services will be delivered within a quality and safety framework.
  • Mainstreamed – we will seek to integrate our service delivery with that of other organisations so that related needs of consumers are met through mainstream service providers.
  • Least restrictive care – we aim to provide our services in a way which allows consumers, as far as possible, to be treated in an environment and a manner that respects their individual dignity, privacy and worth.
  • Evidenced based best practice – we value the use of evidence to inform and improve our practices. We focus on intervention methods that are best practice as based on available evidence and have proven outcomes.
  • Early intervention – we will intervene as early as possible and deliver treatments in a timely manner in order to ensure the best possible outcomes for consumers.
  • Skilled and supported multidisciplinary workforce – we will continue to develop and train our multidisciplinary workforce to build our capacity and ensure more effective service delivery.
  • Population based planning and service delivery – services will be delivered as close as possible to where consumers live.
