Letter to Community Pharmacies - Tasmanian Opioid Pharmacotherapy Program - New Dosing Forms

Dear Sir/Madam

I am writing to you to provide a further update on the dosing sheets that your pharmacy completes on a monthly basis to record pharmacotherapy dosing for clients who are registered on the Tasmanian Opioid Pharmacotherapy Program.

Unfortunately, there was an issue with accessing the monthly summary form online.  This has now been rectified and the forms are available to download on our Community Pharmacy Program webpage

There are now four separate medication administration charts to record dosing information for methadone syrup, methadone liquid (Biodone Forte), Buprenorphine (Subutex tablet) and Buprenorphine (Suboxone film).  To provide a degree of flexibility, each medication administration chart is available in three different formats – PDF, Excel and Word.  Choose the format which best suits your needs.  Further information on the use of the medication administration charts is included in a fact sheet which is also available online at the address above.

It is important to note that the monthly summary report component will form the basis of a cover sheet which will need to be submitted (along with the client monthly dosing sheets) to the Pharmaceutical Services Branch for processing as per usual practice.  Completed dosing sheets can either be faxed to  (03) 6173 0820 or scanned and emailed to pharm.services@dhhs.tas.gov.au

As indicated previously, all community pharmacies are requested to commence using the new forms from  1 August 2016 to ensure timely payments of incentives.
