A referral to the Palliative Care Service can be made by patients, carers/family members or health professionals. It can be made by phone, fax, letter, or email.
A patient can be referred when they:
- have been diagnosed with a life-limiting illness;
- are aware of their diagnosis; and
- agree to be referred - or if the person is a child, or not competent to provide agreement, agreement must be sought from the person’s nominated person responsible.
Referrals can be made in a range of circumstances, including when health professionals, families and carers need support to meet patients’ needs or families and carers need emotional or bereavement support.
The Palliative Care Service will need to contact the patient’s general practitioner or specialist to ensure that the referral is appropriate for the person’s illness and needs.
Referrals are made to an individuals local palliative care service care team. The care teams hours of operations and contact details are provided below.
Hours of operation
The palliative care service operates during the following business hours Monday to Friday.
- Hobart
8.00am to 4.30 pm - Launceston
8.00am to 4.30 pm - Burnie
8.30am to 5.00 pm

Out-of-hours
The Palliative Care Service offers an out-of-hours telephone support service to patients and carers who have been admitted to the Service. Health Professionals can also contact the out-of-hours telephone support service.
The out-of-hours service provides advice and support on symptom control problems such as pain, nausea, vomiting and other urgent issues that are not able to wait until the next day.
