Care Management Guidelines

The Symptom Management Guidelines are a resource for all health professionals caring for palliative patients. They include information about symptom assessment, symptom management, clinical decision making, including setting goals of care, and when to seek specialist advice.

The guidelines and accompanying Consumer Fact Sheets were developed with funding made available through the Australian Government, Local Palliative Care Grants Program.

Inpatient Integrated End of Life Care Pathway: Based on the Liverpool Integrated Care Pathway for the Dying, this tool has been developed by Dr Jurate Kantvilis to use in inpatient settings, and is recommended for use in conjunction with the Terminal Care Guideline.
