About us
Polish Association in Hobart Inc is the auspicing body for the Home and Community Care (HACC) funded services provided through the Polish Welfare Office, with funds received via DHHS.
The Polish Welfare Office has 5 HACC programs:
- Polish Welfare Worker Program
- Polish Outreach Program
- Polish Day Centre Program
- Statewide Telephone Information and Referral Service
- Polish Senior Liaison Officer Program
How to contact us or make a booking
Office hours:
Monday to Friday 9.00am - 2.00pm
Welfare workers: 25hrs per week
Outreach workers: 38hrs per week
Day Centre: Twice a month on the first and third Tuesday of each month
Polish Seniors Liaison Officer: 5hrs per week, Wednesday's only

Service Eligibility
Referrals accepted from clients, families, GPs and agencies, provided the client has given permission for referral.

HACC fees for Day Centre - $2 per session/per day (this fee can be waivered)
Donation for transport
